Add-on Testing

Two Simple Steps

Do you need to add an additional test to a sample already drawn? No worries, follow these two steps:

Step 1: Verify the Validity

To verify the validity, view the Quick Reference Directory to verify the sample is still viable.

Step 2: Submit the Add-On Form

If the sample is still viable, download and fill out the Test Add-On Form. Please do not save the form as a different type of file. Only PDF documents will be accepted. Once this document is filled out and saved on your computer, fill out your name and contact information below (NOT THE PATIENT’S) so that we can contact you if we have a problem with the file or we aren’t able to add on the test. Next, browse for the file on your computer or simply drag it into the upload box below. Once completed, click the “Upload” button – we’ll take it from there.
