Add-on Testing

Two Simple Steps

Do you need to add an additional test to a sample already drawn? No worries, follow these two steps:

Step 1: Verify the Validity

To verify the validity, view the Quick Reference Directory to verify the sample is still viable.

Step 2: Submit the Add-On Form

If the sample is still viable, download and fill out the Test Add-On Form. Once this document it is filled out and saved on your computer, browse for the file or simply drag it into the box below, and then click the “Upload” button – we’ll take it from there.
